Invite family, friends, neighbors, church members, etc. to be a part of our Adoray care team with our Hospice Volunteer Application Form. | | Hospice Volunteer Charting Forms – Tips to Complete Adoray Volunteer Charting forms are designed for hospice volunteers to record interactions of any kind with patients and/or family members. Phone calls, transportation, deliveries, Veteran Recognitions, along with visits are to be recorded. These documents become part of the permanent patient medical record. It is essential that volunteers who provide direct patient services understand the legal and moral implications of such personal medical data. 1. The need for strict confidentiality is essential. Keep all patient information in a secure location. This includes the patient profile/face sheet, assignment forms, copies of completed charting forms, and etc. 2. Record all interaction and attempted interaction. The volunteer notes, along with other Team members' reports, are filed concurrently, and provide a well-documented report of comprehensive hospice patient service. Your documentation can help us notice patterns in patient care. By sharing that you attempted to visit the patient, but they were not home, they were asleep, or not in their room, shows that interactions are being attempted. Something that can be viewed by those regulating our records. 3. Please use a separate documentation sheet for each visit and for each patient. Or, if sending documentation via e-mail, please send a separate email for each patient. This keeps patient “A’s” information from not being scanned (at our office) with Patient “B’s” data, which would violate confidentiality rules. 4. The Volunteer Charting form is a record of your observations. These are legal forms and are part of the patient's permanent medical record. When writing observations: - - use nonjudgmental, descriptive words to record them;
- - avoid personal opinions or ideas on these observations;
- - be concise and state only that which is pertinent and factual.
5. Service Start and End Time: Travel time and mileage are important to include as they provide Medicare with information on cost savings. Be sure to include the date of your visit, the time you left your home, and the time that you returned home from your visit. 6. Activity: check the boxes to indicate all of the services that you provided while with the patient and/or family. 7. Notes: Summarize your observations and conversations. In this area, include your observations of the patient for an overall record of the demeanor (e.g., alert, confused, uncomfortable, sleepy, etc.) of the patient. If the family expresses any concerns, doubts, fears, or needs to you, please note them. Remember, however, that you are stating their concerns, not yours, nor your interpretation of their concerns. You may need to call the Volunteer Coordinator or nurse (including the on-call nurse) if, in your opinion, the need to be met is immediate. 8. Date of next planned visit: This is easy to skip, but it is important to leave a visit knowing when you plan to return, even if it is an estimated date. This helps in preventing weeks from going by until your next visit. Also, it helps the patient/family be aware as part of good communication to them. (Note: Remember to also communicate on the patient’s calendar and/or Adoray notebook.) 9. Volunteer hours are reported to the Board of Directors and to Medicare. Remember, "if you did not write it down, it did not happen". 10. Keep current and write your notes down after each visit, then mail them to the Volunteer Coordinator within seven days of your visit. 11. Destroy all forms (copied or saved documentation forms, patient profile/face sheet, etc.) immediately upon the death of the patient or if the patient is taken off Adoray service. 12. If in doubt about how to complete these forms, please call the Volunteer Coordinator for assistance. Thank you! | | | | | Upcoming Education Opportunities: - Thursday, October 3, 2019 10:00 a.m. – 11:30 a.m. Woodwinds Hospital, Woodbury

When visiting the River Falls store you will notice the whimsical denim aprons hanging though out the store. These are wonderful little works of art are created by our very own volunteer, Marina Johnson. Marina found us after one of our volunteers, Barb O’Malley, suggested she try volunteering at Treasures. Marina didn’t want to cashier and Barb assured her there would be plenty of other tasks to do. Marina thought she would give it a try. She met with Volunteer Coordinator, Jenny Hennemann, and as they say in the movies, the rest is history. We soon learned that Marina had a great talent for sewing and a love for fabric and jewelry. Her great eye for detail and color sense made her a natural. She started repairing clothing items that needed just a minor repair to make them saleable. She then started helping ready items for store display and getting inventory ready behind the scenes. Then one day after receiving a donation with a couple of aprons inside we looked at the number of unsaleable jeans we receive, and the proverbial light bulb went off. “What if we could save those jeans from the rag bin and create something useful and make some money for Adoray? Hmmmm??? We had the how, now we just needed the who. We gathered a few random jeans and excess cotton fabric scraps and showed them to Marina. We asked her if she had any interest in making a few aprons and lucky for us she said “yes!’. Embellishing them with buttons, lace and adding her special touch each apron is unique. She has created close to 50 aprons for the store. Folks have purchased them for bridal showers, birthday gifts and just for the fun of it. Who can’t use a cute little apron? Our customers love them! We love them! They are fun and speak to one of the many offshoots of Treasures - turning folks unwanted goods into meaningful Treasures benefiting Adory’s patients and their families. | | | | On top of stairs apron models, Marcia Lopez, staff member, Volunteer, and Apron creator Marina Johnson, Volunteer Brenda McDaniels and Lisa Aurandt, staff member | | Marina has been sewing since she was a young girl. As a single mom, sometimes it was out of necessity that she sewed for herself and for her three children when they were growing up. She now creates items for friends and her 6 grandchildren. When not sewing or volunteering Marina and her husband Duane love to travel. They have traveled throughout the world. Each trip is an adventure for them. When asked her favorite country to visit Marina answered, “so far it has been Spain. It is truly beautiful there. But Duane and I have many other places on our list.” We wish Marina and Duane many years of adventures and travels, and thank her for sharing her time and talent with us at Treasures River Falls. | | | | | Our Osceola Treasures store celebrated 3 years in July! Here's a picture of store employees Sonya, Joyce and Debbie. Their anniversary week followed with great daily specials, drawings and treats. It was a great week of celebrating and promoting Adoray in the Osceola community. | | | | | Let's celebrate a great summer season at Osceola Treasures! An abundance of donations to the linen department, limited workspace at Treasures II found volunteers struggling to keep up. The solution is more volunteers but the workspace is cramped and people are busy. The solution? Take the donations to “occasional” volunteers in a location with space to spread out and work. A church fellowship hall is spacious, has tables to work on and is usually available. First Reformed Church became the site of a fabric folding party on Tuesday night, July 16th. Church members plus some friends efficiently measured, folded and priced four large bins of fabric. This was an enjoyable evening of visiting and working together to support a worthy cause. Those who participated indicated that they are willing to do this again and a couple even indicated they might become “official” Treasures volunteers. If other churches or service organizations would like to help out in this way, contact us. It is an activity well-suited to fabric measuring and folding, processing sheets, tagging doilies, and more. | | | | | Virtual Suggestion Box Do you have ideas on how to make Adoray a better place to work and care for patients?
